안녕하세요
Today I'd like to introduce an interesting case(several years ago) and, explain my routine protocols for orthognathic surgery.
오늘은 몇 년 전 진행했었던 흥미로운 증례를 하나 소개하며, 악교정수술을 준비하고 마무리하는 프로토콜을 포스팅하고자 합니다.
[Problem list]
Skeletal class II malocclusion / Anteroposterior discrepancy
-Severely protruded maxilla (상악의 돌출)
with Midline deviation / Canting / Yawing
-Retrognathic mandible (하악의 후퇴)
with #35,45,36,46 missing / Severe overjet
Application of 3D Simulation Surgery to orthognathic Surgery for more predictable results, especially in the complex case.
위와 같은 복잡한 턱교정 수술 증례에서는,
수술전에 3차원 가상수술을 진행하는경우,
예측가능한 결과를 얻을 수 있습니다.
Protocols
0. [Pre-surgical orthodontic treatment. -> Orthodontist]
1. [Clinical examination / CBCT taking / Impression & Dental model fabrication Before surgery.]
2. [3D laser scanning of the dental model -> STL export]
3. [Merging of the dental STL with 3D CBCT]
치아 스캔모델을 CBCT 치아에 중첩합니다.
4. [STO- Simulation surgery]
Maxilla
- Segmentation of Mx at #14,24 area (Y segment - #14,24 extraction)
- Re-arrangement of Maxillary arch (SB of Anterior segment)
상악의 돌출을 없애기 위하여 전방 분절을 뒤로 넣습니다.
(하악의 전방이동이동양을 줄이는 효과도 있습니다.-
과두에 전달되는 무리한 힘을 최소화 하는데 좋은 전략입니다.)
- Midline / Cant / Yaw correction
Mandible
- B-SSRO : Advancement (Final occlusion - correction of severe overjet)
- Double sliding advancement Genioplasty

안녕하세요 선생님. 이 수술은 Materialise 사의 Mimics 라는 프로그램을 이용하여 예전에 진행했던 증례입니다. 상기 프로그램이 가격이 워낙 비싸서, 현재는 web 상에 많이 나와있는 여러가지 free 3D program 을 혼합해서 사용하고 있습니다. 기회가 되면 관련 무료로 진행할 수 있는 관련 posting도 향 후에 한번 올리도록 하겠습니다.감사합니다.^^
