Important Notice and Guide for Doctor Members of WebCeph
When a member entrusts the processing of personal information (hereinafter referred to as "patient information") of the person (hereinafter referred to as "patient information") who visited the hospital or clinic operated by the member when using the WebCeph service to AssembleCircle, the member must comply with the following obligations as a consignor of personal information processing in accordance with related laws such as the Personal Information Protection Act.
This is important, so be sure to read it carefully.
1. Obligation to acquire personal information collection and consent
When a member collects patient information from a patient, consent for collection and use of personal information must be obtained from the patient (Article 15 of the Personal Information Protection Act and national laws). If the member does not legally obtain consent to collect and use the patient information from the patient, the patient information must not be entrusted to AssembleCircle for the purpose of using the WebCeph service. Before entrusting patient information to the WebCeph service, be sure to check whether you have obtained consent for collection and use from the patient in advance.
A sample form of consent form to collect personal information from the patient is shown below. ( If the personal information component, purpose, and retention period differs, please modify and use accordingly).

2. Obligation to disclose or consent to the details of personal information processing consignment
For members located in other countries excluding Korea, the EU, and the United States, when using the WebCeph service, patient information collected by the member is transferred and processed outside the country, and it is necessary to obtain explicit consent from the patient.
Therefore, please select the member's country of residence and obtain consent as follows.

3. Other obligations
As a member of the personal information processing consignor under the Personal Information Protection Act, the member should educate the consignee to prevent the personal information of the subject is not lost, stolen, leaked, altered or damaged due to consignment, and should supervise whether the consignee handles the personal information safely, such as checking the processing status. AssembleCircle, as a personal information processing consignee, is striving to establish the highest level of patient information protection system in order to prevent the member's legal responsibility from occurring due to the consignment of patient information.
